Required Forms for Mixed Martial Arts, Boxing and Kickboxing

For promoters:

Licence Application Form (PDF file 157K)
To be completed by event promoters or managers. 

Professional Boxing or Kickboxing Contest or Exhibition Report Form  (PDF file 113K)
To be completed by the promoter and submitted to the Office of the Athletics Commissioner within three days following the boxing/kickboxing event/exhibition.

Professional Mixed Martial Arts Contest or Exhibition Report Form (PDF file 113K)
To be completed by the promoter and submitted to the Office of the Athletics Commissioner within three days following the mixed martial arts event/exhibition.

For fighters:

Licence Application Form (PDF file 157K)
To be completed by fighters. 

Initial and Annual Examination Form (PDF file 143K)
To be completed and signed by a fighter’s doctor. This form must be submitted together with the Licence Application Form.

For all other participants: Licence Application Form (PDF file 157K)
To be completed by officials, seconds, matchmakers and any other person working with the fighter.
